Debit Card (Must have a validity of more than 3 months remaining.)
Credit Bureau Report (NCB Report) issued by the bank, not older than 3 months from the document date

For Monthly Payment via Automatic Bank Transfer

Copy of the bankbook (page showing account name) for direct debit
Auto Debit Application Form or screenshot of the registration transaction
Supporting documents such as proof of income, government officer ID, NCB Report, or pay slips (any one of these)
